Billing & Insurance

We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.

Once provided with complete and accurate insurance information, we will submit claims to your insurance carrier . Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts which are due at the time of service. We accept Cash, Check, Visa, MasterCard, Discover and American Express.

Your Check In / Registration Process:

Please plan to arrive 15 minutes early to update your patient information. You will need to bring your insurance card, copayment and photo ID when you check in. Please bring a list of your current medications. If you are scheduled for an injury which is work related or the result of a motor vehicle accident please bring all the required insurance information which would include the name of the carrier and claim number.

Appointments:

Our office hours are by appointment only. When calling for an appointment please furnish our staff with the reason for your impending visit. This will allow us to give priority to your needs and reserve sufficient time for your visit. When acutely ill, please call the office as early as possible.

If you are unable to make your scheduled appontment, please let us know at least 24 hours in advance.

Repeated missed appointments is a pattern we can not ignore. This behavior is an immense inconvenience to our patients as well as our staff. You may be subject to a missed appointment fee of $25.00.
