A Patient-Centered Medical Home is a team-based model of care, led by a personal physician who provides continuous and coordinated care throughout a patient's lifetime. Our PCMH practice is responsible for providing for all of a patient's health care needs or appropriately arranging care with other qualified professionals. Our team of health professionals, coordinated by a personal physician, works collaboratively to provide high levels of care, access and communication, care coordination and integration, and care quality and safety.
Quaker Medical is recognized both regionally and nationally for its quality. We are a member of The Primary Connection and Optimum Physician Alliance, two elite groups of western New York primary care offices dedicated to quality and cost awareness. We have also attained the highest level of Patient Centered Medical Home (PCMH) as recognized by the National Committee for Quality Assurance.
At this time we are not accepting new adult patients.
We have returned to our full operating hours Monday thru Friday. We are still closed on Saturdays, but our on-call doctor is still available all weekend for urgent concerns. We appreciate your patience while we are working through the rescheduling process.

Quaker Medical is committed to high quality patient care and ensuring safety. We are continuing to provide care via telemedicine (video and phone care) for sick visits such as fever and cough. We are offering select problem visits in addition to well care. We are aiming to see our higher risk individuals for their face to face well care in a setting where they don't have to worry about high risk.
The process of entering the building and proceeding to your visit will be very different from our normal state of affairs. We will contact people in advance of their visits for a pre-screening process as well as for the intake that would normally occur at the front window. The waiting room will not be open. We apologize for the inconvenience, but your car will serve as the waiting room.
We ask that people remember to maintain social distancing while at Quaker Medical- including at the doorway. We ask that regardless of the community rules that may change for facemask wear, that anyone over the age of two wear a face mask while at Quaker Medical. If a patient arrived without a mask, you will be asked to reschedule your appointment or Quaker Medical can provide you with a mask for a fee of $5.00 which you be billed for.
We kindly request, no one walk into the office unannounced/unplanned for any reason. Please call for any needs, including samples and forms.
