A Patient-Centered Medical Home is a team-based model of care, led by a personal physician who provides continuous and coordinated care throughout a patient's lifetime. Our PCMH practice is responsible for providing for all of a patient's health care needs or appropriately arranging care with other qualified professionals. Our team of health professionals, coordinated by a personal physician, works collaboratively to provide high levels of care, access and communication, care coordination and integration, and care quality and safety.

As of June 2021, the vaccination rate of the eligible population in Erie County is over 50% and in Orchard Park it is up to 70%. We are lucky to have seen a string of days where positive Covid testing results have been less than 1% in Erie County. Given this safer situation, we would like to move toward "normalcy" in our scheduling and ability to see sick patients on-site. We will still offer telemedicine for those that request or require it. As we have immune compromised patients, newborns and others in our Quaker community who rely on us to help protect them from Covid, we need all people coming into the office to continue to mask and use social distancing. Please continue to use the phone and patient portal whenever possible. Remember, not every issue requires face to face on-site interaction. We will continue to monitor our patients entrance into the building to assist in maintaining distancing and sanitation. Please continue to wait in your car upon arrival for your appointment until our clinical staff reach out to you. Thank you all again for your patience!
