NCQA RECOGNITION - In February 2014 Quaker Medical Associates achieved Level 3 Recognition (the highest level) for Patient Center Medical Home (PCMH) from the National Committee for Quality Assurance (NCQA).
A Patient-Centered Medical Home is a team-based model of care led by a personal physician who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes. Our PCMH practice is responsible for providing for all of a patient's health care needs or appropriately arranging care with other qualified professionals. It is a model of practice in which a team of health professionals, coordinated by a personal physician, works collaboratively to provide high levels of care, access and communication, care coordination and integration, and care quality and safety.
Quaker Quits Program - For the year 2014, we would like to prioritize smoking cessation with our "Quaker Quits" program. If you are a smoker, we would like to help you succeed at permanently quitting. 2013/2014 Flu Season: Quadrivalent flu vaccine is still available in our office for ages 6 months and older. It is ideal to be vaccinated early, the flu shot can still be helpful later. Flu season lasts from October to May, so it is still a good idea to get protected.
